CHECK THE WELL-BEING

On January 31, 2018 at approximately 1445 hours, Cst OHASHI reviewed an e-mail string from PMPD's Professional Standards Section to advise they had recieved an e-mail from the Office of the Police Complaints Commissioner (OPCC) written by Daniel BERLADYN in which he sent the following remarks to the OPCC email account:

"There is a very likely chance that I will end up choosing to commit suicide as I have been in holding for months..."

"Do you want me to just disappear?"

"I drop my belongings off there (his mother's house) in a plan to get into hospital before I end up killing myself."

"... which can only mean they want to drive me into suicide."

According to the OPCC, this e-mail was sent by Daniel BERLADYN on the evening of January 30, 2018.

As per PRIME, Daniel BERLADYN was last known to be residing at the Salvation Army's Caring Place located at 22188 Lougheed Highway in Maple Ridge, BC. A cell phone ping revealed a location of Dewdney Trunk Road and Cardinal Street in Mission, BC, with a 109 meter radius.

CPIC messages to be sent to Ridge Meadows RCMP and Mission RCMP to conduct patrols for Daniel BERLADYN in order to assess his well-being.

On June 22, 2018 at 1115 hours, Cst. MANDERSCHEID assisted Cst. OHASHI with a check wellbeing of Daniel BERLADYN. Cst. MANDERSCHEID was informed that BERLADYN was making suicidal comments in court earlier in the day and it was necessary to check his wellbeing. Cst. MANDERSCHEID attempted several calls to BERLADYN to arrange a meet in person, but all calls were negative. Cst. MANDERSCHEID could not ping BERLADYN's phone as the number listed on PRIME were not accurate at the time. A CPIC request was sent to the Ridge Meadows RCMP to assist in locating BERLADYN.

INVESTIGATIVE FOLLOW-UP:

On June 22, 2018 at approximately 1610 hours, Cst OHASHI liaised with Cst MANDERSCHEID regarding the status of this investigation. Cst MANDERSCHEID confirmed with Cst OHASHI that all telephone numbers listed on PRIME for Daniel BERLADYN have been called with negative results (including an additional telephone number provided by [22][22] which was also found to be out-of-service). As these telephone numbers all appeared out-of-service or were no longer associated to Daniel BERLADYN, a ping has not been done. Cst OHASHI updated [22][22] on today's court outcome [22] at approximately 1158 hours and confirmed at this time that [22] believed Daniel BERLADYN to no longer have an active cell phone.

Cst MANDERSCHEID has also confirmed that he has sent an e-mail to Daniel BERLADYN in an attempt to locate him and ensure his well-being.

Cst OHASHI notes that Ridge Meadows RCMP attended the Salvation Army in Maple Ridge and were informed by staff that Daniel BERLAYDN has not stayed at the shelter for the past one (1) month. As the shelter staff advised that Daniel BERLADYN often frequents the Maple Ridge Public Library, this location was also checked by the Ridge Meadows RCMP with negative results.

At approximately 1549 hours, Cst OHASHI phoned Maple Ridge Community Corrections and spoke with Daniel BERLADYN's probation officer who advised that Daniel BERLADYN last checked in on June 19, 2018, but that staff did not have any information about his whereabouts. The probation officer confirmed that Daniel BERLADYN was no longer staying at the shelter and had mentioned he was couch surfing in the past. The probation officer did suggest checking the Tim Horton's located on Lougheed Highway and 224th Street in Maple Ridge, BC, as Daniel BERLADYN is known to access wi-fi from this location. A CPIC message has been sent regarding same.

Daniel BERLADYN has already been added to CPIC as a "Compassion to Locate" with a request to assess his mental health.

Cst OHASHI has also requested that a LMD BOLF be sent regarding Daniel BERLADYN with a request to assess his mental health when located.
