On August 23, 2017 at 1544 hours [blank] contact S/Sgt CARROLL to stated that [blank] Daniel BERLADYN had attended [blank] residence and made suicidal comments. Daniel BERLADYN has a long history of Mental Health related issues and blames [blank] for many of his personal problems. Daniel BERLADYN stated "take a good look at me because it's the last time you'll see me. I'm going to kill myself" then drove off in his vehicle (BCLP HY9995). Daniel BERLADYN has no cellular phone that is active (query shows the # on PRIME is now a land-line). Area patrols were negative. CPIC sent to CQ RCMP to attend Daniel BERLADYN's residence and check on him. LMD BOLO sent regarding vehicle and matter.
