FORM 4 APPREHENSION:

On February 16, 2016 at approximately 1639 hours, Cst OHASHI spoke with Tri-Cities Mental Health who advised that a psychiatrist was in the process of certifying (completing a Form 4) Daniel BERLADYN who was currently at their center located at 2232 Elgin Avenue in Port Coquitlam, BC. While Tri-Cities Mental Health indicated they initially called the Coquitlam RCMP to transport Daniel BERLADYN to hospital, they re-contacted Cst OHASHI advising Daniel BERLADYN had left their facility before police arrived. At approximately 1810 hours, Cst OHASHI, Cst QUEENAN and Cst WONG attended Daniel BERLADYN's residence located at 560 Foress Drive in Port Moody, BC, and apprehended Daniel BERLADYN without incident.

Daniel BERLADYN was transported to Royal Columbian Hospital where another psychiatrist admitted him into the Psychiatric Emergency Department.

NARRATIVE

On February 16, 2016 at approximately 1639 hours, Cst OHASHI spoke with Tri-Cities Mental Health who advised that a psychiatrist was in the process of certifying (completing a Form 4) Daniel BERALDYN who was currently at their center located at 2232 Elgin Avenue in Port Coquitlam, BC. While Tri-Cities Mental Health indicated they initially called the Coquitlam RCMP to transport Daniel BERLADYN to hospital, they re-contacted Cst OHASHI advising Daniel BERLADYN had left their facility before police arrived (X-Reference Coquitlam RCMP File: 2016-5047).

At approximately 1810 hours, Cst OHASHI, Cst QUEENAN and Cst WONG attended Daniel BERLADYN's residence located at 560 Foress Drive in Port Moody, BC, and apprehended Daniel BERLADYN to Royal Columbian Hospital and met with Cst OHASHI who provided background information to staff. As Daniel BERLADYN presented as emotionally labile and agitated while at Royal Columbian Hospital, Cst OHASHI, Cst QUEENAN and Cst WONG all remained at the scene for officer safety reasons.

At approximately 2045 hours, Daniel BERLADYN was certified by another psychiatrist and held in the Psychiatric Emergency Department.

On February 16, 2016 at approximately 1807 hours, Cst QUEENAN and Cst WONG attended 560 Foress Drive with Cst OHASHI to assist with a Form 4 Apprehension of Daniel BERLADYN. Daniel was cooperative with police and apprehended without incident. Cst QUEENAN then transported Daniel to Royal Columbian Hospital (RCH), the transport began at 1812 hours and concluded at 1826 hours. Police stayed with Daniel without incident until he was admitted into the Psychiatric Emergency Department by a psychiatrist at approximately 2046 hours.
