This file was created to provide the best possible care and delivery of policing services for Daniel BERLADYN who is actively followed by the Port Moody Police Department's Mental Health Liaison. For background information, Daniel BERLADYN has been apprehended by police under Section 28 of the Mental Health Act or via a Form 4 Warrant a total of three (3) times since January 2016 for both suicidal and homicidal ideation. Furthermore, Daniel BERLADYN is currently awaiting disposition for a Peace Bond Application (X-Reference PMPD File: 2016-616) and a Section 111 Firearms Prohibition Application has been submitted for Crown Counsel for charge approval (X-Reference PMPD File: 2016-982).

The Port Moody Police Department's Mental Health Liaison will be monitoring Daniel BERLADYN and and documenting interventions using an electronic data set, however, will enter into this PRIME file any officer safety issues or significant information that may be able to assist members in responding to calls for services involving Daniel BERLADYN. At such a time the Port Moody Police Department's Mental Health Liaison is no longer actively monitoring Daniel BERLADYN, a text page will be entered into this file advising fo same.
